Getting started with Hopewell in-home care for veterans

Many families of veterans hesitate to seek help, not wanting to impose or unsure how benefits apply. But the care your loved one has earned is there for moments like these.  

Hopewell makes accessing VA-covered in-home care for veterans straightforward and stress-free, with a team that understands military service and the importance of maintaining dignity and autonomy at home. 

Eligibility depends on enrollment in VA health care and a clinical need for assistance with daily tasks. Veterans of any age who meet certain eligibility requirements may qualify if a VA provider determines that help with activities of daily living (ADLs) is needed. 

Veterans who aren’t signed up for VA health care but believe they’re eligible for coverage can apply at the VA website, by phone at 877-222-8387, by mailing an application for health benefits form, or in person at a VA medical center or clinic. 

We guide you every step, from initial call to seamless implementation, so the focus stays on comfort and connection. 

Here’s the process for getting started with Hopewell in-home care services:

    Assessment: A VA provider evaluates the veteran’s needs to determine if they qualify for assistance.

    Care plan: If the veteran is approved for assistance, a plan is developed outlining the type and frequency of support. Make sure to request the authorization to provide services be sent to Hopewell so care can begin.

    Home visit: A Hopewell care coordinator meets with the veteran at home to finalize the care plan, answer questions, and ensure the veteran feels comfortable with the upcoming support.

What is the VA Home Health Aide Program?

The VA Home Health Aide program offers no-cost personal care services for eligible veterans, helping with daily tasks such as hygiene, mobility, and meals so they can continue living independently at home.

What areas do you serve?

We provide veteran care services through our four offices across North and Central Florida, working hand-in-hand with the VA to deliver personalized, in-home support. Our team can help confirm availability in your area.

I’m a veteran. How do I qualify?

Veterans who served in the active military, naval, or air service and didn’t receive a dishonorable discharge may qualify if a VA provider determines that help with activities of daily living (ADLs) is needed. Veterans who are not already signed up for VA health care but believe they meet eligibility requirements are encouraged to apply online.

Is this benefit free for eligible veterans?

Yes! This care is completely covered by the VA for eligible veterans as part of the care they’ve earned through their military service.

How often can services be provided?

The frequency of care is based on the veteran’s needs and what is authorized by the VA, ranging from a few hours a week to more regular support.

How do I get started?

The first step is to contact your VA provider or social worker to discuss your needs. Your provider will then complete an assessment to determine if you meet the eligibility requirements and help coordinate services if approved. Be sure to let them know you’d like to use Hopewell, so they can send the service authorization directly to our team.
